Voluntary Life Insurance Supplemental life insurance provides extra life insurance protection for you, your spouse, and your dependent children up to age 26. For this year’s Open Enrollment, you will not be required to complete an Evidence of Insurability (EOI) form if you elect up to the Guaranteed Issue Limit of $200,000 of coverage for yourself or $50,000 for your spouse.
