You should call your Doctor’s office for instructions if you need care in the evenings, on weekends, or during the holidays and cannot wait until the office reopens. If you have an Emergency, call 911 or go to the nearest Emergency Room. If you are experiencing a mental health crisis, you may also call 988 for assistance. Out-of-Network Services When you do not use a Tier 1 or Tier 2 In-Network Provider or get care as part of an Authorized Service, Covered Services are covered at the tier 3 Out-of-Network level, unless otherwise indicated in this Booklet. For services from a Tier 3 Out-of-Network Provider: 1. The Out-of-Network Provider can charge you the difference between their bill and the Plan’s Maximum Allowed Amount plus any Deductible and/or Coinsurance/Copayments unless your claim involves a Surprise Billing Claim; 2. You may have higher cost sharing amounts (i.e., Deductibles, Coinsurance, and/or Copayments unless your claim involves a Surprise Billing Claim); 3. You will have to pay for services that are not Medically Necessary; 4. You will have to pay for non-Covered Services; 5. You may have to file claims; and 6. You must make sure any necessary Precertification is done. (Please see “Getting Approval for Benefits” for more details.)
