Notice Regarding Tobacco Cessation 7 REASONABLE ALTERNATIVE STANDARD (RAS) PROGRAM OPTIONS 1. 1-800-QUIT-NOW: Indiana’s Tobacco Hotline (4 Free Calls, 10 if Pregnant) Sign your company up at quitnowindiana.com/healthy - workplace • Quit Now Preferred Employers (must register online) receive: • Ability to refer employees to the Indiana Tobacco Quitline • Materials to educate employees about tobacco use and the Quitline • Updaters on new resources, research and Quitline promotional materials • Monthly summary of the number of employees who were referred to the program (Due to HIPAA regulation, specific employee data cannot be provided) 2. Quit Now enhanced program support of the Indiana Wellness Council ($2/PEPY, $199 min, $2,500 max) • Web-based program built as a complement to the services of 1-800-QUIT-NOW • 12 Chapters (10-15 minutes to participate ea. Chapter) with a 5 day restriction between chapters • Reporting capability offered so designated coordinator of the account can pull user reports anytime to help in incentive completion tracking 3. Freedom From Smoking®, by the ALA – Four Program Delivery Options • Online – Self-paced course includes 9 interactive sessions split into 3 phases ($99.95/participant) • Onsite with a Small Group – offers personalized attention and peer support. 7-week program led by ALA trained facilitators. Available in English &Spanish ($4,000 for 8-16 employees, includes course materials) • On Your Own Interactive Guide – can be used on its own or as a supplement • By Phone – telephone counseling services from certified counselors specializing in smoking cessation 4. Health Plan Tobacco Counseling Coverage • Anthem - CPT codes 99406 and 99407 • Tobacco Cessation – Members can earn up to $40 for quitting smoking through the Quitline. They receive $20 to sign up and another $20 upon completion of the program. • United Health Care - CPT codes 99406 and 99407 5. Wellness Portal Tobacco Cessation Programs – Various Portal Providers 6. Health Coaching – Focused on smoking cessation 7. Action Plan with a Physician - Verification of the plan should be generic if employer will be collecting it or if using a 3rd Party Vendor to collect detailed plan to adhere to all compliance standards.
