Eligible hip and knee surgeries and revisions at one of these Designated Orthopedic Network facilities will be covered under the employees' benefit plan, with normal cost-sharing (such as deductibles or coinsurance) applying. To receive these benefits, procedures must be performed at one of the Designated Orthopedic Network facilities.
