Basic Life Insurance The basic life insurance policy is available to all full-time employees. In the event of your death, your beneficiaries will receive a lump-sum payment based on your employee classification. HCCS pays all but $1.00 of the total annual premiums for these benefits. Basic Accidental Death & Dismemberment Insurance The basic AD&D insurance policy is available to all full-time employees. In the event of death, loss of (or loss of use of) a body part or function, speech, eyesight, or hearing, your beneficiaries will receive a lump-sum payment based on your employee classification. This benefit is bundled with Basic Life. Long-Term Disability Disability benefits protect your income during a period in which you are unable to work because of an illness or accident not related to your job. HCCS pays all but $1.00 of the total annual premiums for these benefits. Benefits begin on the 90 th day after the date of the incident and will cover 60% of your earnings (up to $5,000/month).
