Life Changing Event

Information that must be provided to add child to your insurance: 1. Legal name of child 2. Date of Birth We will also need the Social Security Number once you receive it. Please call before the 30 day window for enrollment. Experiencing a Life Changing Event It is your responsibility to notify your insurance account when this event happens, and you want to make a change to your coverage plan. You have 30 days from the event. Please be prepared to supply the necessary documentation for the life changing event. Make the change by signing into your PlanSource Account and requesting a Life Changing Event. https://benefits.plansource.com/logon Having a Baby It is your responsibility to notify your insurance account when your child is born and/or adopted, and you would like to add the child to your insurance coverage plan. You have 30 days from the date of birth to enroll your child. Please provide the social security number once you receive it in the mail to benefits @goshenschools.org.
