What Is Not Covered The Surest Plan does not provide Benefits for the following services, treatments, or supplies (including items or services that are related to the services, treatments, or supplies listed below, but which are not specifically listed below) even if they are recommended or prescribed by a Provider or are the only available treatment for your condition, unless specifically described or listed in Section 5.1 (Covered Health Services). Alternative Treatments 1. Acupressure. 2. Acupuncture. 3. Aromatherapy. 4. Art therapy, dance therapy, horseback therapy, music therapy, and other forms of alternative treatment as defined by the National Center for Complementary and Alternative Medicine (NCCAM) of the National Institutes of Health. 5. Biofeedback. 6. Health care services ordered or rendered by Providers or para-professionals unlicensed by the appropriate regulatory agency. 7. Holistic medicine and services, including dietary supplements. 8. Homeopathic or naturopathic medicine, including dietary supplements. 9. Hypnotism. 10. Massage therapy that is not physical therapy or prescribed by a licensed Provider as a component of a multi-modality rehabilitation treatment plan. 11. Rolfing. 12. Vocational therapy. Behavioral Health: Mental Health/Substance Use Disorder 13. Services that are solely educational in nature or otherwise paid under state or federal law for purely educational purposes. 14. Inpatient or intermediate or outpatient care services that were not pre-authorized. 15. Investigational therapies for treatment of Autism Spectrum Disorders. 16. Non-medical 24-hour withdrawal management which is an organized residential service, including those defined in the American Society of Addiction Medicine (ASAM) criteria providing 24-hour supervision, observation, and support for patients who are intoxicated or experiencing withdrawal, using peer and social support rather than medical and nursing care. 17. Outside of an initial assessment, services as treatments for a primary diagnosis of conditions and problems that may be a focus of clinical attention but are specifically noted not to be mental disorders within the current edition of the Diagnostic and Statistical Manual of Mental Disorders by the American Psychiatric Association.
