Wellness Benefit Overview for Accident Insurance

ACCIDENT INSURANCE Wellness Benefit Did you know your plan includes our Wellness Benefit1? This benefit was created to promote annual screenings and we encourage you to take advantage of the benefit offered. The benefit pays according to a fixed schedule once per covered person, per calendar year and it pays in addition to your other coverages. DocuSign® is online e-signature platform that enables you to submit claims without any printing, mailing, faxing or scanning. We simply send the form to your current email, you can fill out the form completely online, and we automatically receive it once you are done. There are nearly 30 tests2 covered, including: • Blood test for lipids including total cholesterol, LDL, HDL and triglycerides • Breast ultrasound or mammography • Chest x-ray • Colonoscopy • Electrocardiogram (EKG) • Echocardiogram (Echo) • Immunizations (such as flu shots or the COVID-19 vaccination) • Interscholastic sports physical exams • Pap smear • PSA (blood test for prostate cancer) How do I submit a claim? We have three easy ways for you to submit your claims: 1. Online at www.sunlife.com/account • Log in to your Sun Life account and select ‘submit a claim’ 2. Via email, fax or mail • Visit www.sunlife.com/findaform • Choose ‘Accident’ • Open ‘Wellness/Cancer Screening’ form and complete • Send in the form – Email: SLFWorksiteclaims@yourbenefitexpert.com – Fax: 866-376-9480 – Mail: Sun Life P.O. Or to request claim forms be sent via DocuSign • Call: 877-820-5306 • State you would like to receive your wellness claim forms via DocuSign • Provide your preferred email address Once we receive your completed form, you will receive your payment very soon. 1. The Accident policy provides one Wellness Benefit per calendar year per covered person from the list of covered benefits. See policy for details of covered items and services. Only tests and procedures listed in the policy are eligible for benefit payment. Some Accident policies do not contain the Wellness Benefit. 2.
