Employees All Full-Time Employees are eligible for the benefits program. You may insure yourself and eligible family members under the program. Spouse & Legal Dependents Your children are eligible for medical, dental, and vision to age 26. Your children of any age are also eligible if you support them, and they are incapable of self-support due to disability. Working Spouse Provision Qualifying Events You may make a change to your benefits if you have a qualified status change such as marriage divorce, birth/adoption, death, changes in spouse’s benefits, and more within 31 calendar days of the event. If you miss the 31-day deadline, you will have to wait until the next benefits open enrollment period to make changes. Eligibility

      How do Rx Benefits work? Medical ID Provide the pharmacy with your medical ID card. Insurance Coverage The pharmacy will run your prescriptions through insurance and the total amount due is owed to the pharmacy. Deductible Reached Once you reach the deductible amount, you will then pay the reduced Rx coinsurance amount based on the tier of your drug. Deductible Contribution The total amount owed will then count towards your deductible. Out-of-Pocket Max Once you reach your total out-of-pocket max, your employer will pay 100%.

      Pharmacy Cost TOOLS GoodRx GoodRx has both a website and a mobile app that can be used to compare prices. Go to the website and type in your drug name. GoodRx will display the cost available at multiple pharmacies. Print the coupon and present to your pharmacist.       Health Savings ACCOUNT A Health Savings Account (HSA) is available to those enrolled in the HSA Qualified Plan.       Watch This HSA Video to Learn More View Qualified Expenses The 2026 annual max contribution is $4,400 for an individual and $8,750 for families. In addition to your own contributions, the University will also contribute to your HSA. Contributions are pre-tax and your maximum contribution is indexed annually by the IRS. You must contribute 25% of the University seed amount via payroll deduction in order to receive the University’s seed contribution.       Doctor Visit Visit your healthcare provider and the office will submit the claim to your health plan. Amount Owed The health plan will share the amount you owe with your doctor. EOB The health plan send you the Explanation of Benefits (EOB). Bill Received Your doctor will then send you a bill. HSA Qualified Plan and HSA Consumer Experience Pay with HSA You can use your HSA funds to pay the bill from your doctor.

      HEALTHCARE FSA LIMITED PURPOSE FSA Flexible Spending Accounts Flexible Spending Accounts (FSA) are set up to pay for many of out-of-pocket medical expenses with tax-free dollars. The FSA account holder sets aside a pre-tax dollar amount for the year used to pay for medical expenses. Unused FSA funds can expire at the end of the year. 1 Healthcare FSA An FSA is an alternative to an HSA. FSAs are typically paired with a PPO plan while an HSA is paired with an HDHP. FSA funds can be used on various medical, dental, and vision related expenses. The IRS contribution maximum for 2026 will be $3,400. 2 Limited Purpose FSA A limited purpose FSA is only for dental and vision expenses. If you elect to participate in the High Deductible Health Plan and contribute to a Health Savings Account (HSA), you qualify for this FSA, not the full healthcare FSA. The IRS contribution maximum is $3,400. Watch This FSA Video to Learn More 1 2 View Eligible Expenses DEPENDENT CARE FSA 3 3 Dependent Care Account (DCA) FSA A DCA is a tax-free spending account for dependent care expenses such as daycare, preschool, or day camps for any dependent under the age of 13 or who is physically or mentally incapable of self-care. The IRS contribution maximum for 2026 will be $7,500 for single individuals and couples filing jointly, and $3,750 for individuals who are married filing separately.

      VOLUNTARY VISION VSP offers two levels of coverage, Basic and Premier, with no changes to premium rates for 2026. The Basic plan is the lower cost option and has copays for exams and lenses; whereas the higher cost option, the Premier coverage plan, has $0 copays and larger allowances for other covered expenses.       Employer Paid BENEFITS Eligible Ball State members will receive employer-paid benefits such as Short-Term Disability and Basic Life Insurance with Accidental Death & Dismemberment. In 2026, these benefits will move from The Hartford to Voya.

      Basic Life Insurance The employer-paid basic life insurance policy is available to all full-time personnel. In the event of your death, your beneficiaries will receive 2x your annual earnings to a maximum of $125,000. Coverage decreases by 40% at age 66. Basic Accidental Death & Dismemberment Insurance The employer-paid basic AD&D insurance policy is available to all full-time personnel. In the event of death, loss of (or loss of use of) a body part or function, speech, eyesight, or hearing, your beneficiaries may receive an additional payment equal to the volume of your basic life insurance policy. Short-Term Disability Service personnel receive a short-term disability benefit protects your income during a short period of time due to illness, maternity leave, or an accident not related to your job. This benefit is subsidized at 75% by the University. Benefits begin on the 8th day after the date of the incident and will replace up to 80% of your lost income per week for a maximum of 26 weeks. Long-Term Disability Employer-paid long-term disability protects your income for a period of time due to illness, maternity leave, or an accident not related to your job. The elimination period will be equal to the maximum duration of your short-term disability policy (26 weeks for service personnel, 13 weeks for all other employees), and will replace up to 60% of your lost income per month until you reach Social Security Normal Retirement Age.

      Accident Insurance Accident insurance is designed to help you meet the out-of-pocket expenses and extra bills that can follow an accidental injury. It can be used however you want, and it pays in addition to any other coverage you may already have.               Voluntary Life Insurance Supplemental life insurance provides extra life insurance protection for you, your spouse, and your dependent children up to age 26. For this year’s Open Enrollment, you will not be required to complete an Evidence of Insurability (EOI) form if you elect up to the Guaranteed Issue Limit of $200,000 of coverage for yourself or $50,000 for your spouse.                       Short-Term Disability The University currently subsidizes STD benefits for Service employees only, which allows employees to bridge the gap between STD and Long-Term Disability (LTD) benefits. Ball State offers Voluntary STD benefits to our faculty, professional and staff employees.
