Benefit Highlights 2025-26 PLAN YEAR DETAILS Eligibility • All full-time employees working 30 hours per week are eligible to participate in Duncan Supply benefits on the 1st of the month following 30 days of employment. • You may insure yourself and eligible family members under the program. • Your children are eligible for medical, dental, and vision to age 26 (and Voluntary Life Insurance where the maximum dependent age is 26) • Your children of any age are also eligible if you support them, and they are incapable of self- support due to disability. Proof of Dependent Eligibility As required by our insurance contracts, you may be required to provide proof of eligibility for your dependents. If your dependent becomes ineligible for coverage during the year, you must contact Human Resources within 30 days. Making Benefit Changes After Open Enrollment You may make a change to your benefits if you have a qualified status change such as: • Change in marital status • Change in a child’s dependent status • Birth or adoption of a child • Death of a spouse, child, or other qualified dependent • Change in your spouse’s benefits or employment status Last Day of Benefits Upon full-time separation of service, benefits will end on the last day of the month of your date of termination. You may also request a printed copy of all notices, free of charge, upon request.
