Medical Plan Duncan Supply Company emphasizes the importance of health, wellness, and financial security for employees and their families. During Open Enrollment, which runs from February 6 to February 14, 2025, employees can enroll in or make changes to their benefits. Open Enrollment for 2025-26 is passive, meaning if no changes are made, current elections will automatically roll over. However, it’s still the only time of year to adjust benefits unless a qualifying life event occurs. Any changes made will take effect on March 1, 2025. Benefit Highlights 2 0 2 5 - 2 6 P L A N Y E A R D E T A I L S Proof of Dependent Eligibility As required by our insurance contracts, you may be required to provide proof of eligibility for your dependents. If your dependent becomes ineligible for coverage during the year, you must contact Human Resources within 30 days. Eligibility • All full-time employees working 30 hours per week are eligible to participate in Duncan Supply benefits on the 1st of the month following 30 days of employment. • You may insure yourself and eligible family members under the program. • Your children are eligible for medical, dental, and vision to age 26 (and Voluntary Life Insurance where the maximum dependent age is 26) • Your children of any age are also eligible if you support them, and they are incapable of self- support due to disability. Last Day of Benefits Upon full-time separation of service, benefits will end on the last day of the month of your date of termination. Compliance The offered plans meet essential and affordable guidelines in compliance with the Affordable Care Act. All Federal and State required plan notices can be found in PlanSource. Home Delivery Pharmacy – 90-day supply (maximum cost share noted above) Maintenance medications are available through Carelon Rx Mail (Ingenio Rx will became Carelon Rx on January 1, 2023). You will need to call us on the number on your ID card to sign up when you first use the service. Specialty Pharmacy – 30-day supply (cost shares noted above for retail and home delivery apply). We may require certain drugs with special handling, provider coordination or patient education be filled by our designated specialty pharmacy. Drug cost share assistance programs may be available for certain specialty drugs. Preventive Drugs - Your pharmacy cost share is waived for in-network drugs included on the Preventive Drug Enhanced drug list, a designated list of drugs to treat health conditions, such as: diabetes, asthma, depression, heart health, high blood pressure, high cholesterol, and osteoporosis.

Simply visit www.goodrx.com, enter the drug name, and GoodRx will show the cost at multiple pharmacies. You can then show the coupon to your pharmacist. Registration and an ID card are not required to use GoodRx. Pharmacy Cost Comparison Tools Prescription drug costs can vary by pharmacy. While your insurance carrier offers discounts when you use your ID card at the time of filling, there are also vendors that may offer additional or greater discounts. You’re encouraged to use available tools to shop and compare prices the next time you need to fill a prescription. Anthem Tools to Know Anthem Member Website Portal Once you receive your Anthem ID card, register on the portal at www.anthem.com. The portal gives you access to your ID cards, claim summaries, and EOB statements. You can search and compare costs of different providers and services, download claims and mail order delivery forms, price medications, and access discount programs available through Special Offers. LiveHealth Online Live Health Online provides immediate doctor visits through live video. Sign up at livehealthonline.com or download the mobile app. Board certified physicians available 24/7, prescriptions available, $59 per visit. Licensed psychiatrists and psychologists are also available at an additional cost. Pharmacy Immunization If your physician has given you a prescription for the flu, pneumonia, or shingles vaccine, you can have that vaccine administered at a participating pharmacy. Visit CVS, Kroger, Meijer, Target, Walgreens, and Walmart and show them your ID card. The coverage is the same as if you were in your doctor’s office. There is no cost to you. Sydney Mobile App Download the mobile app to get a copy of your ID card immediately via your phone (which can be faxed or emailed to your provider); compare costs of nearby doctors for different procedures; search for in- network doctors, urgent care centers or hospitals; check the cost of drugs or get refills; and view your mobile Health Record and share it with your doctor. 24-Hour NurseLine Anthem has you covered by offering a 24/7 nurseline. Visit your Anthem portal and select Discounts to view the member exclusive offers. BCBS Global Core When you travel outside the U.S., your Anthem group plan covers you in the event of an emergency. Finding an in- network PCP is easier with our online tools. You can search for a doctor by name or look for one near you. Avoid getting care from doctors outside your plan because it will likely cost you more, or your plan may not cover it at all. See a board-certified doctor 24/7 – You don’t need an appointment to see a doctor. They’re always available to assess your condition and send a prescription to the pharmacy you choose, if needed. It’s a great option when you have pink eye, a cold, the flu, a fever, allergies, a sinus infection or another common health issue.
