Payment is generally due monthly on the first day of the month. Payment is considered made on the postmarked date. You will be given a grace period of 30 days within which to make the payment. Termination of the Continuation Coverage. The USERRA continuation coverage may be terminated before the end of the maximum continuation period for any of the following reasons: • The employer no longer provides group health coverage to any of its employees. • The premium for USERRA continuation coverage is not paid on time (including the grace period). • Termination for cause under the generally applicable terms of the Surest Plan (e.g., submission of fraudulent benefit Claims).
