Your cost, which is based on your age and benefit volume, will be available in PlanSource when you login to enroll. • Voluntary AD&D coverage: Benefit is automatically added for all employees and spouses who elect Voluntary Life coverage; the AD&D amount is the same as the elected Voluntary Life amount. • New Voluntary Life elections, less than the guaranteed issue (GI) amount, or STD/LTD elections, elections, made during the 2025-26 open enrollment period, do not require evidence of insurability. • New voluntary life elections or increases to current elections, over the GI amounts, will require an Evidence of Insurability (EOI) Application and approval by New York Life.
